Notes

The omop_note table captures unstructured information that was recorded by a provider about a patient in free text (in ASCII, or preferably in UTF8 format) notes on a given date. The omop_note table contains free-text causes of death from the Medical Certificate of Cause of Death (MCCD), origin and format of death record from the death registry.

1	encoding_concept_id	integer	A foreign key to the predefined Concept in the Standardized Vocabularies reflecting the note character encoding type.
2	language_concept_id	integer	A foreign key to the predefined Concept in the Standardized Vocabularies reflecting the language of the note.
3	note_class_concept_id	integer	A foreign key to the predefined Concept in the Standardized Vocabularies reflecting the HL7 LOINC Document Type Vocabulary classification of the note.
4	note_date	date	The date the note was recorded.
5	note_datetime	timestamp	The date and time the note was recorded.
6	note_id	integer	A unique identifier for each note.
7	note_source_value	integer
8	note_text	string	The content of the Note.
9	note_title	string	The title of the Note as it appears in the source.
10	note_type_concept_id	integer	A foreign key to the predefined Concept in the Standardized Vocabularies reflecting the type, origin or provenance of the Note.
11	person_id	integer	A foreign key identifier to the Person about whom the Note was recorded.
12	provider_id	integer	A foreign key to the Provider in the PROVIDER table who took the Note.
13	visit_detail_id	integer
14	visit_occurrence_id	integer	Foreign key to the Visit in the VISIT_OCCURRENCE table when the Note was taken.
