Notes
The omop_note table captures unstructured information that was recorded by a provider about a patient in free text (in ASCII, or preferably in UTF8 format) notes on a given date. The omop_note table contains free-text causes of death from the Medical Certificate of Cause of Death (MCCD), origin and format of death record from the death registry.Further details can be found in the OMOP CDM Clinical Data Tables specifications - https://ohdsi.github.io/CommonDataModel/cdm53.html#NOTE
