This document details the rationale and procedure for administration of the health and wellbeing web-based questionnaire for UK Biobank.

The overall aims of this questionnaire were:

• to capture data on possible symptoms and impacts of long COVID;
• to enable comparisons of symptoms experienced by participants who have objective measures of previous COVID-19 disease (obtained through medical or vaccination records) with those who do not.
