Objective This study aims to investigate the association between the triglyceride-glucose (TyG) index and incident fracture risk, as well as bone mineral density (BMD), in a large prospective cohort from the UK Biobank. Methods We enrolled 337,631 participants without fractures at baseline in the UK Biobank. Cox proportional hazards models, along with multivariate linear regression, were utilized to assess the relationships between the TyG index, fracture risk, and BMD. Restrictive cubic splines (RCS) were employed to analyze the nonlinear associations of the TyG index with fracture risk and BMD. Results During a median follow-up of 12.3 years, 18,549 all fractures and 3041 hip fractures were documented. The hazard ratios (HRs) and 95% confidence intervals (95% CIs) in intermediate versus the low TyG index were 0.77 (0.68-0.87) and 0.86 (0.82-0.90) for hip fractures and all fractures, respectively. Contrarily, the HRs (95% CIs) in high versus the low TyG index were 1.06 (0.77-1.48) and 1.05 (0.91-1.22) for hip fractures and all fractures, respectively. We observed that higher TyG index was significantly associated with increased BMD at femur trochanter (β = 0.0093 ± 0.0016, P = 8.86E-09) and total femoral (β = 0.0104 ± 0.0017, P = 3.85E-10). Furthermore, the results of RCS analysis revealed a U-shaped association between TyG index and fracture risk, and an S-shaped association with BMD at multiple sites. Conclusions The elevated baseline TyG index was associated with a lower risk of fracture, in a U-shaped fashion. An S-shaped association between TyG index and BMD at multiple sites was also observed.</p>
